The CCD contains your: Allergies, Current Medications, Current Health Issues, Test Results and Immunizations as you see them in MyChart. You can put this information on a USB drive and you can take it with you wherever you go and give it to other organizations where you're seen for care.
In addition to your health information you can view information about recent outpatient visits and hospital admissions. Recent Emergency Department visits will be available on the Recent Visits page. For each visit you can view a summary report, which contains information like vital signs, patient instructions, etc. For admissions you will see a list of past hospital admissions, which include: the date(s) you were admitted and discharged and the unit to which you were admitted. If you click on an admission you can view the discharge instructions that were given to you.
Test results can be released to MyChart by your provider. We are now also automatically releasing some test results after 4 or 14 days, depending upon the test result. * In order to view your test results, they must be released to you by your healthcare provider.
